  1. All attendees must wear a mask before entering and while inside the building including the rooftop. (A face shield can be worn in addition to the mask).

The only area of the Shul currently open is the main sanctuary for Friday night services and Shabbat day, and the rooftop for Shacharit and Minch / Ma'ariv.  All other areas are currently closed to public traffic. Please note there is limited access to washrooms. While on the premises, please do not walk around; remain in your seat. If you cannot stay in one place, please do not come to the services.

  2. Anyone who is over the age of 65, and if you are immunocompromised, or has another condition that increases the risk of severe illness with COVID-19 is strongly urged to continue to daven at home and to consult with your physician to determine whether it is a good idea to join a minyan.

  4. A cleaning plan for high-touch areas is in place

(A face shield can be worn in addition to the mask, but it is mandatory to wear a mask.)

I agree to each statement above and understand that attending any minyan at The Village Shul is my choice.  I therefore release The Village Shul and Aish HaTorah Learning Center and its directors, officers, employees, agents, volunteers, sponsors and representatives from any and all liability for the unintentional exposure or harm due to Covid-19.  Moreover, if my answers to any of the questions on the survey change between the time I complete it and the service for which I have signed up for, I will contact eh Shul and I will not attend until I have heard back from The Village Shul that it is safe to do so.
I would like to attend the following services:
